how do we interpret widal test to diagnose typhoid fever?
Q.

A. This test is now outdated. there are better tests. However, generally, a titre of 1: 60 or more is supposed to be diagnostic for endemic areas, otherwise, 1: 80 or more. Rising titre is more significant, i.e. when you repeat the test after a few days, the titre is more. Widal is positive late in the first week of illness onwards. Initially, O antibodies rise and later, F antibodies are more. F antibodies indicate previous infection or vaccination.

A. The Widal test is an in vitro test whereby bacteria causing typhoid fever are mixed with serum containing specific antibodies obtained from an infected individual. It is a presumptive serological test for enteric fever or undulant fever. In case of Salmonella infections, it is a demonstration of the presence of O-somatic and H-flagellar antigens in the blood. For brucellosis, only O-somatic antigen is used. The Widal test is not a very specific test, since patients are often exposed to other bacteria (e.g. Salmonella enteritidis, Salmonella typhimurium and some types of E. coli) that induce cross-reactivity; many people have antibodies against these enteric pathogens, which also react with the antigens in the Widal test, causing a false-pos [cont.]

A. S.Typhi H and S.Typhi O are the two kinds of antibodies you would produce if you were exposed to typhoid, or if you were recently vaccinated for it. To see if antibodies are detectable they do repeated dilutions of your blood until you can't get a reaction at all. the proportion of blood in which there is a reaction is called a titer. They are shown as normal ranges in a fraction like S.Typhi - H 1/160 and S.Typhi O-1/40. Those fractions are the normal ranges, in other words if you have less than that of each kind, you are fine. You did not include the part that would indicate what the results of the test were, if they are negative you do not have typhoid... but even if they are positive you may not have typhoid, and will then get another… [cont.]

A. Widal test is an agglutination test for the detection of agglutinins (antibodies) for H and O antigen for salmonella in patients with enteric fever. Procedure: Serial dilutions of the patients serum is taken from 1:10 to 1:640. To each equal volumes of Salmonella antigens are added. The tubes are incubated overnight and read. Result: The highest dilution of the patients serum in which agglutinations occurs is noted, ex. if the dilution is 1 in 160 then the titer is 169. A single test of O titer of 1:100 or more and of H titer of 1:200 or more is significant. A rising titer of four fold or higher in an interval of 7 - 10 days is more meaningful than one test. Cautionary factors: In endemic areas, low titer of agglutinins is present in the… [cont.]
